Lou Brady

Since 1999, Lou Brady has been on a mission—a mission to create access to healthcare for everyone. That mission brought Lou to the Cornell Scott-Hill Health Center in 2011 when he took on the newly created position of Director of Operations and Quality Improvement. After successfully designing and implementing some fundamental operational changes, Lou was promoted to his current position—Chief Operations Officer.

Lou brings an impressive array of healthcare leadership experience to his current role, including over 10 years of community health center experience. Most recently, Lou served as the Executive Director of the Geiger Gibson Community Health Center in Boston. Prior to that, he was a Site Director for Community Health Center, Inc. in Meriden, Connecticut. Lou’s community health center experience, however, began at the Lowell Community Health Center in Lowell, Massachusetts.

In 1999, Lou was working as the Director of Ambulatory Services for Jewish Memorial Hospital in Boston. That’s when he met Joe Feaster, then President of the National Association of Community Health Centers. Additionally, Mr. Feaster served as both a board member for Jewish Memorial Hospital and as board chair for the neighboring Dimock Community Health Center. It was Mr. Feaster who encouraged Lou to “join the movement,” explaining to him how community health centers are a living legacy of the civil rights movement, when people dedicated their lives to ensuring access to healthcare for all and eliminating health disparities. That held a lot of appeal for Lou, and he joined the Lowell Community Health Center as soon as an opportunity became available.

Now, more than 10 years later, Lou is using his passion and experience to make a difference in the lives of the people served by the Cornell Scott-Hill Health Center. While Lou’s accomplishments to date are numerous, he knows that there is more work to be done and looks forward to doing what is needed to provide the health center’s patients with the experience they need and deserve. And thanks to Lou’s hard work and dedication, the health center is now one step closer to doing that.
Lou recently spearheaded the health center’s efforts to achieve patient-centered medical home recognition, which was met with much success. As a patient-centered medical home, the health center will be able to provide its patients with personalized care, including a care team that will allow for easier access to care.

In response to the 2017 Department of Social Services Person Centered Medical Home Plus (PCMH+), Lou developed an agency value-based care model, created a new Care Coordination Department and an internal PCMH+ Project Team to provide oversight and management to serve the 10,281 Medicaid patient panel. CS-HHC has successfully satisfied PCMH+ Wave 1 and will be participating in PCMH+ Wave 2.

In Partnership with the Community Health Center Association of Connecticut Inc., Lou led the agency Practice Transformation Network (PTN) in a collaborative team effort in Transforming the Clinical Practices Initiative. CS-HHC received the Transformation Award 2017, for Commitment to Care Team Development & Optimization, presented by Community Health Center Association of Connecticut.
Lou has developed and implemented a center-wide population health management approach resulting in increased care integration, provider productivity, patient satisfaction, and Healthcare Effectiveness and Data Information Set HEDIS clinical quality scores.

Lou serves on the Senior Leadership Team of Cornell Scott-Hill Health Center. He collaborated with Senior Leadership, Fair Haven Community Health Center, and Yale New Haven Hospital to develop the New Haven Primary Care Consortium, a coordinated citywide system of primary care delivery.
Lou enjoys spending time with his family, reading and weight training. An avid reader, Lou spends hours haunting local bookstores in search of his next read.

Lou received his B.S. degree in Industrial Management from the University of Massachusetts at Lowell in Lowell. He received an M.B.A. in Business Administration from Boston University in Boston.
