Birdies for Charity!

Jul 26, 2016

It’s that time again; time for the annual Travelers Championship Golf Tournament. Held every summer at the TPC River Highlands in Cromwell, the tournament features exciting golf action as well as fundraising opportunities for nonprofits like us!

The Birdies for Charity fundraising program is a pledge based program based on the number of birdies (a score of one stroke better than par) made by the PGA TOUR professionals during the week of the Travelers Championship. Donors can pledge per birdie (e.g., if 1,500 birdies are made, a $.02 pledge would equate to a $30.00 donation to our organization) or for a specific amount. Donors may also guess the total number of birdies that will occur during the Tournament. The donor who guesses closest to the actual number of birdies without going over the total will win a $1,000 gift card and a $500 donation for their charity.

Pledges and donations may be made online. For more information, please contact Rose Swift (rswift@cornellscott.org).

This year’s Travelers Championship will be held August 1st through the 7th. To learn more, please visit: http://travelerschampionship.com/.

GROW! Up with Good Nutrition

Oct 29, 2015

Being a community health center means going above and beyond to meet all of your patients’ needs…not just the medical, dental or behavioral health ones. That’s why our Early Childhood and Wellness departments have partnered with Connecticut Food Bank to bring its GROW! Up with Good Nutrition program to several health center families. The program is comprised of two parts: workshops and a mobile food pantry. The workshops, which are presented by the SNAP-Ed Nutrition Program and Read to Grow, cover topics such as healthy eating and financial literacy. Participating families attend the workshops then have the opportunity to shop for healthy food on the program's special GROW! (Grocery On Wheels) Truck. All of this is at no cost to participants. The truck visits the health center every other week and will do so for the next six months. We are very excited to bring this program to our patients and their families and want to thank the Connecticut Food Bank and the Our Family Foundation by Stop & Shop, whose grant made this possible!

Health Center Receives Generous Grant From the CVS Health Foundation

Apr 24, 2015

The health center has some exciting news to share! We recently found out that we are the recipients of a generous grant from the CVS Health Foundation, a private foundation created by CVS Health Corporation. The health center was one of fifty-five grant recipients selected as part of the Foundation’s multi-year, $5 million commitment to increase access to health care in communities nationwide.

The grant we received is in support of shared medical appointments, a collaborative program between Wellness and Internal Medicine, that aims to lower A1c scores in diabetic patients whose scores are 9 percent or higher.

The grants were awarded through the CVS Health Foundation's partnerships with the National Association of Free & Charitable Clinics (NAFC), the School-Based Health Alliance and the National Association of Community Health Centers (NACHC). Grants awarded in partnership with NACHC and the "Innovations in Community Health" program, which includes our grant, are to support the development of innovative, community-based programs that focus on the treatment and management of chronic illnesses. Our health center was one of only two health centers in Connecticut to receive a CVS Foundation grant.

Birdies For Charity

Apr 24, 2015

Now that winter is behind us, our thoughts are turning to summer; specifically we are thinking about this year’s Travelers Championship Golf Tournament. Held every June at the TPC River Highlands in Cromwell, the tournament features exciting golf action as well as fundraising opportunities for nonprofits like us!

Pledges and donations may be made online or by completing a pledge card. For more information, please contact Rose Swift (rswift@cornellscott.org).

Up next is the Chip in For Charity Program, which works like this: we will be selling discounted tickets to the Travelers Championship. Tickets can be purchased for $20.00, a savings of up to $19.00. Give Kids A Smile Day 2014

Jan 28, 2014

We are thrilled to announce that the health center will be participating in this year’s Give Kids A Smile Day.

Give Kids A Smile Day is an initiative of the American Dental Association that aims to connect children to free oral healthcare and to establish dental homes for them. The program, which began as a one-day event in February 2003, has grown to include local and national events year-round. Dentists and other team members volunteer their time, and services, to provide screenings, treatments and education to children throughout the United States. Each year, approximately 450,000 children benefit from more than 1500 events, all because of the efforts of 40,000 or more annual volunteers.

The Cornell Scott-Hill Health Center is proud to be a part of Give Kids A Smile Day 2014. Appointments are available on Saturday, February 8th from 8:30 a.m. to 12:30 p.m. at our Derby, Connecticut care site located at 30 Elizabeth Street in Derby.

Individuals interested in receiving free dental services through the Give Kids A Smile program should be 18 years or younger and need to have a parent or legal guardian accompany them. Appointments are necessary and can be made by calling 203-736-5460.

The Cornell Scott-Hill Health Center’s Richard O. Belden Dental Clinic opened in 2008, filling a critical need in the Valley by providing affordable, quality dental care to area residents. Since opening, the office has served over 14,000 Valley residents. Additionally, it has had 33,000 encounters (visits) during those five years, and for the past three years has averaged almost 1,000 new patients per year.

Join the Fight to End Childhood Lead Poisoning

Dec 10, 2013

From the City of New Haven:

In 2011, the City of New Haven Health Department’s Bureau of Environmental Health, assisted 151 children whose blood lead levels were at or above 10 micrograms per deciliter. Over the past 17 years, the number of reported cases of childhood lead poisoning in the City of New Haven has declined, from 420 children at or above 20 micrograms per deciliter reported in 1995 to 119 children at or above 10 micrograms per deciliter in 2011. This success was only achievable by the implementation of a comprehensive Lead Based Paint Hazard Control Program made possible with assistance provided by the United States Department of Housing and Urban Development (HUD), together with the involvement of New Haven residents and homeowners. With your help, the number of pediatric lead poisoning cases will continue to decline.

The City of New Haven has received $2,480,000 from HUD of which, $1,620,000 shall be made available over a three year period to provide funding to homeowners to undertake lead hazard control activities in private rental housing units. Applications are now being accepted from property owners for 0% deferred loans to assist with the eradication of lead hazards. These deferred loans are completely forgiven after five (5) years; therefore, borrowers are cleared of any financial obligation.

For details regarding this program and how to apply, please contact the New Haven Health Department’s, Bureau of Environmental Health at (203) 946-8174 or click here for more information.

This past Wednesday Senator Chris Murphy (D-CT) stopped by to talk with patients and staff about the Affordable Care Act. The Senator first met and spoke with CEO Michael Taylor as well as the Access to Care Team. Their discussion focused on the new healthcare law and how the health center is helping people navigate and understand their choices when it comes to coverage. Murphy also had the opportunity to speak with patients who now have health insurance after enrolling through Access Health CT, Connecticut's Official Health Insurance Marketplace. Then he sat down with one patient as she went through the enrollment process. Collaborative Wellness Initiative Featured On News Channel 8!

Oct 23, 2013

During Tuesday’s 5:30 p.m. broadcast, News Channel 8’s Jocelyn Maminta shined the spotlight on “The Weigh to Live,” our collaborative wellness initiative with Chapel Haven. Jocelyn attended “The Weigh to Live” program on Chapel Haven’s Whalley Avenue campus earlier in the day and spent a significant amount of time there, observing students and conducting interviews. Rose and John, two of Chapel Haven’s students, discussed their experiences with the class as well as what they have learned. Chapel Haven’s President, Michael Storz was also interviewed as was our very own Jill Meyerhoff, who is overseeing the program.

The program, which is designed to help adults with developmental and social disabilities achieve and maintain a healthy lifestyle, was rolled out earlier this year and achieved encouraging results. Of the fifteen students who attended the spring class, twelve students lost a combined sixty-two pounds. Eleven students achieved lower blood pressure readings. Six students lowered their cholesterol. And six students lost inches in their waist or hips.

"Weight management and fitness are vital components of leading a happy and healthy adult life," said Michael Storz, President of Chapel Haven. "We have been delighted with the success of this initiative. The staff of the Cornell Scott-Hill Health Center are remarkably skilled at making fitness fun and manageable for our students and that is the key to any sustainable health program."

Due to the program’s success it is being offered again this fall. This semester there are 14 students enrolled. To date, they have lost a combined twenty-six pounds. The program is also expanding to include students from Chapel Haven’s Asperger’s Syndrome Program. That class is set to begin next week.

The program was created with the following goals in mind: to help Chapel Haven’s clients attain and maintain a healthy weight; to reduce clients’ risk of chronic disease; and to help clients live a healthy lifestyle. To achieve those goals, health center staff created an eleven week program that covers everything from weight loss tips to healthy snacks. Weigh-ins, food tastings, hands-on activities and food demonstrations are also included in the weekly classes.

As previously mentioned, the program is being overseen by Jill Meyerhoff, a Cornell Scott-Hill Health Center Wellness Educator:

“We are so pleased with the results of this program. While the students are losing weight and inches as well as lowering their blood pressure and cholesterol, it’s really the immeasurable results that we are most proud of. For example, the students have been buying new, healthier foods at the grocery store and learning how to portion control their foods. These are important skills that will serve them well as they move toward independent living.”

If you missed the News Channel 8 story, you can check it out here.

Joining Forces with the Mobile Health Window

Jun 3, 2013

On Friday May 17, our Wellness Education and Outreach staff embarked on a new journey – with the Consulate General of Mexico out of New York. We have joined forces to assist the Consulate’s mobile units with their health initiative – Mobile Health Window – Ventanilla de Salud Movil. This new program aims to facilitate access to information, primary health services, and promote a culture of prevention and participation in health related practices among Mexican immigrants in the tri-state area.

The Mobile Health Window visits different locations in the area, 3-4 days at a time 9-1:30. Approximately 60-100 different people attend each day, utilizing the services offered – among which our staff offered blood pressure and blood sugar screenings as well as general information about the health center. We captured almost ½ of the participants and plan on attending as many of the events as we can in the future. We strongly believe this is a wonderful opportunity and collaboration for both Cornell Scott-Hill Health Center and the Consulate General of Mexico!

The grant has allowed the health center and CMHC to partner in the opening and operation of the Wellness Center, a person-centered medical home for CMHC’s clients. Located at CMHC’s main building in New Haven, the Center was created to help improve the health of adults with serious mental illness while also enhancing CMHC clients’ experiences of care and reducing medical costs. CS-HHC staff will provide an array of services including primary care, routine screenings, health education and care management.

The celebration of this new partnership and opening of the Wellness Center will be held on Monday, February 25th. The ribbon-cutting ceremony will take place on the second floor of CMHC at 10:00am and will be followed by a reception.

Meet the Health Center's New OB/GYN Chief

Apr 9, 2012

We are pleased to announce the appointment of Conwell B. Carter, MD as Chief of Obstetrics and Gynecology.

Dr. Carter, a Board certified obstetrician and gynecologist, joins the health center from Los Angeles, Californiawhere he most recently served as the Associate Clinical Chief of Obstetrics at Cedars-Sinai Medical Center. In addition to his administrative role at Cedars-Sinai, Dr. Carter maintained a private practice in theLos Angeles area for over forty years, during which time he delivered 5,896 babies.

Throughout his career, Dr. Carter has also maintained a focus on patient education, giving lectures on such topics as oral contraceptives and hormone replacement therapy. He has won several awards including theUniversityofCalifornia, Los Angeles Medical Students Core Clerkship Teaching Award in 2007 and 2008, and the Cedars-Sinai Health Associates Patient Satisfaction Award for 2006 and 2007.

“Dr. Carter’s appointment highlights the quality of care as well as the caliber of providers available at not only our health center but at community health centers across the nation,” said Jamesina Henderson, the health center’s Chief Executive Officer.

Dr. Tom McNamee, the health center’s Chief Medical Officer added:

“It is always a good day when you have a physician of Dr. Carter’s stature join your team. It’s obviously a benefit to the health center but more importantly it’s a benefit to our patients. That’s what matters most.”

“There are many factors that I took into consideration when weighing this opportunity; however, my desire to make a difference in a population that is typically underserved far outweighed any other factor,” said Dr. Carter. “I am thrilled to be here and cannot wait to start seeing patients.”

Dr. Carter received his bachelor's degree from the University of Pennsylvaniaand his medical degree from the Howard University College of Medicine. He completed his obstetrics and gynecology residency at the U.S. Naval Hospital,San Diego.

In addition to Dr. Carter, the health center is welcoming another new face to the Department of Obstetrics and Gynecology, although this is a familiar one. Sue Ortoleva, a Certified Nurse Midwife, worked at the health center for ten years prior to joining a private practice six years ago.

“When I worked at the health center previously, I felt a sense of satisfaction knowing that in the course of my day I may have changed someone’s life if even in just a little way. It was that feeling that brought me back to the health center,” said Ortoleva.

Ortoleva received her nursing degree form Grace-New Haven Nursing School and her certification in nurse midwifery from theUniversity of Arizona.
