Applications for Access Health CT
Access Health CT is Connecticut’s healthcare exchange. Through the accesshealthct.com website, Connecticut residents can select and sign up for health insurance. The ACCESS to Care Team can help you navigate your healthcare options through Access Health CT, help you select the best option for you, and assist you in registering for healthcare.
Medicaid-A,B and D
Medicaid is a healthcare program offered to individuals whose income and resources are insufficient to pay for healthcare. The ACCESS to Care Team can help you determine your eligibility for Medicaid and assist you in signing up for benefits.
Sliding-fee Discount Program
The Cornell Scott-Hill Health Center seeks to make its services more affordable for anyone who qualifies based on income and family size. Typically the discount program benefits individuals who are uninsured, have high deductibles, or have low income. The ACCESS to Care Team will help you determine your qualifications and assist you in applying for one of five discount levels.
Where can I find the ACCESS to Care Team?
The ACCESS to Care team is available Monday through Fridays from 7:00 am to 5:00 pm at the Cornell Scott-Hill Health Center West Haven Health Center on 285 Main Street. The team is also available at the following health center care sites.

• 911-913 State Street, New Haven
• 121 Wakelee Avenue, Ansonia

What do I need to bring with me to my appointment?
When you call to schedule your appointment, we will let you know everything that you will need to bring. These items may include:
• Your last four (4) consecutive pay stubs (or 2 if paid bi-weekly)
• Taxes for the current year
• Photo ID (birth certificate for minors)
• Proof of residency
• Social Security Card
• Permanent Resident Card
