Payment Options

Request Medical Records
To request your medical records, you can start by contacting our Medical Records Department, either in person or through a designated request form. You may be required to provide identification and complete necessary authorization forms to validate your request.
Sliding Fee Scale
Our sliding-fee discount program allows patients who qualify to pay a reduced fee for many of the Health Center’s services. The Eligibility and Enrollment Team will help you determine your qualifications and assist you in applying for one of five discount levels.
