Patients treated at Cornell Scott-Hill Health Center can request a copy of their medical records by faxing, emailing or mailing a signed Authorization for Access/Release of Information form, as indicated on the authorization. You can also access this information via MyChart.

Important: When filling out your form, be as specific as possible with what information you would like from your medical record. Be sure to include your/the patient's date of birth, current address, current phone number and dates of service.

 

Medical records can be obtained in-person by visiting our Release of Information Department located at 428 Columbus Avenue, New Haven, CT. Please be sure to bring proof of identification with you when obtaining medical records. Our hours of operation are Monday, Wednesday, and Friday between 8:00 am and 5:00 pm.

  • What if my child or someone living in my household already has insurance?

     Include them on the application! All members of your household are factored into your application regardless of whether or not they have coverage already. 

  • How long is the sliding fee scale?

    One year from the beginning date to the end date. 

  • Can anyone apply for the sliding fee scale?

    Yes! Anyone can apply. Your income and the number of people in your household determine eligibility. 

  • When will I know if I’ve been approved for the sliding fee scale?

    Same day! Once we have all of your information, we can let you know the same day if you qualify and you can begin using your benefits immediately. 

  • Who sees my private information?

    Only us. All of the information you provide remains confidential at Cornell Scott-Hill Health Center.

  • Can I use the sliding fee scale with other hospitals or providers?

    No. The Cornell Scott-Hill Health Center sliding fee scale only applies to our services and providers.  Check with your institution to see if they offer their own program.

  • Can you help me apply for medical assistance?

    Yes! Cornell Scott-Hill Health Center can assist you with many programs and guide you on whom to call for Medicare and Medicaid C.
