A sliding fee scale is a cost that can be changed or adjusted based on certain factors. At Cornell Scott-Hill Health Center, the sliding fee scale adjusts the cost of services based on:
Household income
Size of immediate family in household including spouses, domestic partners, children, parents of children living in household together; children 19 years or older on their own if they are not a full time student.
Grandparents and mother- or father-in-laws living in the same home need to apply and be processed separately from the above household members.
At Cornell Scott-Hill Health Center, anyone can apply for a sliding fee scale to determine the cost paid for services. All applications will be processed using the U.S. Department of Health & Human Services 2020 Poverty Guidelines. There are 5 programs that an applicant can qualify for depending on income and family size.
The process is simple and easy. All inquiries can start with a phone call and the rest can be done via email. (Technical assistance available for email communication.)
All family members should be included on the application regardless of whether or not they have insurance.
The sliding fee scale applies to all services and lowers the cost of medical payments, lab work, and prescriptions.
The sliding fee scale is valid for one (1) year from the date of approval.
Your sliding fee scale applies to all Cornell Scott-Hill providers including primary and specialists.
All application information collected is private and confidential and is only stored in the Cornell Scott-Hill Health Center database. This information is not shared with any third party.
An approved application is valid only at Cornell Scott-Hill Health Center and is not transferable to other institutions that may offer their own sliding fee scale.
