Community-based relationships and strategic collaborative partnerships with community groups, stakeholders and educational institutions are vital to our innovative approach to healthcare. These collaborations provide expanded opportunities to invest in our communities. Our research is more impactful because of the partnerships formed throughout the years. Our commitment to community outreach has been the foundation of our mission for the past
fifty-five-plus years. Since our inception, the Community Outreach Initiative has evolved to
encompass a comprehensive range of services to meet the diverse healthcare needs of our
community. We are dedicated to fostering the well-being of every individual by providing
screenings, addressing Homeless Healthcare and combating Infectious Diseases. We supply
information on essential services such as Women's Health, WIC (Women Infants and Children),
Pediatrics and Behavioral Health.
Mobile School-Based Health provides medical and behavioral health services to students and introduces the services to our communities.
Our School-Based Health Centers (SBHC) have expanded to include community outreach as an
integral avenue to educate parents and students on the services available and the benefits to each.
We have recently expanded our school-based health program into Hamden and West Haven and
continue to seek additional opportunities. SBHCs assure that students, particularly those uninsured and under-insured, have access to comprehensive health and preventative services needed to be healthy in school and ready to learn.
