At Cornell Scott-Hill Health Center, our mission has always been to deliver state-of-the-art, integrated, and compassionate care to our community. We are thrilled to announce our involvement in the Connecticut AHEAD (States Advancing All-Payer Health Equity Approaches and Development) initiative, which is set to transform health equity policy across the state in 2025.
Partnering for Health Equity
The Connecticut AHEAD program is a groundbreaking initiative funded by a $12 million award from the Centers for Medicare & Medicaid Services. This program aims to improve population health, reduce disparities in health outcomes, and manage the rising costs of healthcare. The establishment of a 21-member advisory committee brings together experts from diverse organizations to develop and implement statewide health equity policies.
Our CEO, Michael Taylor, is honored to represent Cornell Scott-Hill Health Center on this committee. This recognition underscores our commitment to being a national leader in community healthcare.
Key Responsibilies For Advisory Committee
- Developing equity targets for the statewide health equity plan.
- Producing annual progress reports to track initiatives.
- Reviewing hospital equity plans and guiding investments.
These goals align seamlessly with our mission to serve as a Patient-Centered Medical Home (PCMH) dedicated to creating a healthier and more equitable future for all.
